      Description

How to Edit/Create Ingest Forms documentation uses as example (text and images) a dc form for which every element name are named like
      "dc:elementname".

Currently (since 2013?) Form builder does not allow "colons" in element names. This validation occurs on JS, so it's impossible to create or edit and save a form with colons in their "identifier" value.

Moreover, if you copy an existing form (created by hand or contributed by anmodule) that has elements using "colons", the included XLSTs (the ones that transform from schema versions) replaces all special characters in attribute names that are not allowed in XML for "". So a form that has colons in element names end with "" in the editor.
